This registration form is only for those wishing to register their child for the morning session of our summer golf camp for the week of July 19-22. The camp will run from 9:00 A.M.- 12:00  P.M.  each afternoon. It might be helpful to show up a few minutes earlier for check in on the first day (Monday).

This is just a waitlist. By putting yourself on the waitlist, there is a chance that you could be added to the camp is someone drops out. 

 I agree that The Shepherd's Church may use such photographs of me and my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
